The foundational principle of Soviet healthcare, a state-directed medical doctrine that prioritizes disease prevention over treatment. Formulated by Nikolai Semashko as the first People's Commissar of Health in 1918, it integrated universal free care, sanitary supervision, dispensarization (dispanserizatsiya), vaccination, and health education into a single state system. Semashko's formulation, 'Prophylaxis is our path, dispensarization is the method for solving preventive tasks,' defined Soviet medicine's identity in distinction from hospital-centered curative medicine, and the principle deeply influenced public health systems in socialist states including Cuba and North Korea.
