Voskhod 1 commander and the first human to perish in a spaceflight

"I cannot orient the spacecraft to the sun" — amid cascading failures aboard Soyuz 1, Komarov continued manual troubleshooting and reporting to ground control until the final descent.

A Soviet Air Force test pilot and aerospace engineer, he commanded Voskhod 1 in 1964, the world's first multi-crew spaceflight. One of only two cosmonauts in the first group to hold an Air Force Academy engineering degree, he contributed significantly to Vostok and Voskhod spacecraft design and to cosmonaut training evaluation. In 1967 he flew Soyuz 1 solo on its first crewed test flight; after re-entry, a parachute failure caused the capsule to crash, making him the first human to die in a spaceflight. His death forced a fundamental review of design verification and safety procedures across the Soviet crewed space program.

Soyuz 1’s first crewed test flight and accident

On 23 April 1967, Komarov flew alone aboard Soyuz 1, the first crewed flight of the new Soyuz spacecraft. During the return from orbit, the main parachute did not deploy; the reserve parachute became entangled with the still-attached drogue chute and failed to open. The descent module struck the ground at about 144 kilometers per hour. According to the official conclusion issued by the accident commission in autumn 1968, the cylindrical container holding the main parachute had deformed inward during depressurization, jamming the parachute. Alternative, unofficial explanations also blamed quality-control failures at the spacecraft factory or incorrect parachute packing; these proposals should be distinguished from the commission’s formal conclusion. The commission recommended replacing the cylindrical container with a conical one, increasing and smoothing its interior, adding an emergency separation system for the main parachute’s drogue, and photographing parachute packs before assembly.
