세마슈코 모델
Semashko Model
소련 초대 보건인민위원 니콜라이 세마슈코가 1918년부터 구축한 단일 지불자(single-payer) 국영 의료체계. 전 국민에게 무료 진료를 국가 예산으로 제공하며, 모든 의료기관과 의사는 국가 소속이다. 참가자치야(участковый, 지구 담당제)에 기초한 5단계 위계로 조직되었으며, 예방의학과 사회위생학을 핵심 원리로 삼는다. 쿠바, 조선민주주의인민공화국, 동유럽 여러 나라가 이 모델을 채택했으며, 세계 최초의 전국민 무상의료 제도로 평가된다.
